ACCEPTABLE DOCUMENTATION TO VERIFY U.S. CITIZENSHIP
YOU MAY SUBMIT A COPY OF ANY ONE OF THE FOLLOWING TYPES OF DOCUMENTATION TO VERIFY YOUR U. S. CITIZENSHIP STATUS:
- A Birth Certificate
- A valid U. S. Passport Book (current or expired)
- A valid U. S. Passport Card (current or expired)
- A Certificate of Naturalization from the Immigration and Naturalization Service (INS)
- A Certificate of Citizenship from the Immigration and Naturalization Service (INS)
- A Report of Birth Abroad of a Citizen of the United States
(Must bear an embossed seal including the words “United States of America” and “State Department”. Acceptable Form numbers: FS-240, FS-545, or DS-1350)
IF YOU DO NOT HAVE ACCESS TO ANY OF THESE DOCUMENTS AND YOU ARE A U.S. CITIZEN, CONTACT THE APPROPRIATE GOVERNMENT AGENCY AND MAKE ARRANGEMENTS WITH THEM TO SEND A COPY OF THE DOCUMENT TO YOU. HERE ARE SOME TELEPHONE NUMBERS TO ASSIST YOU IF YOU NEED TO CONTACT ANY OF THESE AGENCIES:

If you are now a U.S. Citizen and became a citizen after you received your Social Security Card, go to the Social Security Administration, provide them your updated U.S. Citizenship status, and request a duplicate Social Security Card.
